    胃食管结合部是食管与胃的交界区域、是食物进入胃的门户，位置深、毗邻重要血管与脏器，该部位手作历术一直是胃肠外科的难点与挑战。该部位间质瘤源于肌层，表面粘膜通常保持完整，这对手术提出了极高要求：既要彻底切除肿瘤，又要尽量减少对胃食管结合部正常管壁层及下食管括约肌的损伤，防止术后胃食管反流、梗阻、漏等严重后遗症。传统开腹手术创伤大、术后恢复慢；单纯腹腔镜手术可能发生辨识食管下端管壁层面不精准导致粘膜损伤、消化道开放、消化液外溢污染腹腔；单纯胃镜必然需要全程切开食管下端，导致消化道开放，还可能发生胃镜难以处理的大出血。而源于肌层的间质瘤源，其表面正常粘膜可以保留不用切除。同时，该肿瘤位于食管下端，该部位还是人体防止胃食管反流的重要结构——下食管括约肌所在部位，一旦手术范围过大，损伤下食管括约肌，则可能会发生胃食管反流这个严重后遗症；手术范围过小，则肿瘤易复发。

    面对如此复杂与两难的情况，彭孟寅医师团队创新采用双镜联合术式，实现了两种微创技术的优势互补与精准叠加：在腹腔镜下精准分离肿瘤假包膜与周围组织，清晰显露食管壁各层结构，筑牢安全防线；胃镜则实时介入，精准监测粘膜层分离情况，指导团队在粘膜下完整剥离肿瘤病灶。这一精妙配合，既确保了肿瘤的根治性切除，又完整保留了胃食管结合部粘膜的完整性，更最大限度减少了对下食管括约肌的干扰，有效避免了传统手术需进行食管胃吻合带来的反流、狭窄等并发症风险，真正实现了肿瘤学根治与功能学保护的双赢。

Today, we are proud to celebrate the smooth recovery and discharge of a patient who underwent a highly complex dual-endoscopic resection of a gastroesophageal junction stromal tumor, performed by Dr. Peng Mengyin’s gastrointestinal surgery team​ at Yibin Second People’s Hospital.

The gastroesophageal junction (GEJ)—where the esophagus meets the stomach—is one of the most technically demanding areas in gastrointestinal surgery. Deeply positioned and surrounded by major blood vessels and vital organs, this region serves as the gateway for food entering the stomach.
